Is Your Neuro-Physiotherapy Working? Five Questions to Ask Your Therapist

When therapy feels like maintenance and you cannot see a change in mobility or daily independence, it is reasonable to ask whether your current plan is working.

After 15 years in the NHS and private practice, I have seen how much the approach to neuro-rehabilitation matters. These five questions can help you have a clearer conversation with your therapist.

TL;DR

  • Ask what your neuro-physiotherapy is intended to improve in daily life.

  • Find out how your therapist checks whether you are making progress.

  • Ask why each exercise is part of your plan.

  • Discuss whether the treatment follows evidence-based neurological rehabilitation.

  • If you cannot see tangible change, ask when the plan will be reviewed.

What Is This? (Short Answer)

This is a five-question checklist for assessing your current neuro-physiotherapy. It helps you ask whether your treatment has a clear purpose, a sound clinical rationale and a way to track progress towards mobility and independence.

You do not need to challenge your therapist to use it. You need answers you can understand.

How does this work?

Bring the five questions to a conversation with your current therapist and ask them to explain your plan in plain English.

  1. What change are we working towards? Ask how treatment connects to your mobility or independence in daily life.

  2. How will we know whether I am progressing? Ask what changes your therapist is looking for and how they will assess them.

  3. Why am I doing these exercises? Your therapist should be able to explain the neurological rationale behind your programme.

  4. How does this approach align with evidence-based neurological rehabilitation? Ask why the chosen treatment is appropriate for your needs.

  5. When will we review the plan if I am not improving? A review gives you a chance to discuss what is happening and what should happen next.

For example, a family may feel caught in a loop of appointments that seem to maintain the same level of function, without a tangible change in mobility. The first step is to ask what progress the current plan is meant to produce and how that progress is being assessed.

Who is this for?

This checklist is for people receiving neuro-physiotherapy and for families trying to understand whether current care is helping.

It may be especially useful if treatment feels repetitive, the purpose of exercises is unclear, or you are not seeing changes in mobility or daily independence. The conversation should centre on your own goals and your therapist’s clinical reasoning.

What does it cost?

The video does not give a price for treatment or for a review of your current plan. Ask your provider directly about any fees before arranging further care.

You can still use these questions in a conversation about treatment you are already receiving.

What are the risks?

The risk raised here is continuing with treatment without understanding what it is meant to achieve or whether it is helping.

A lack of obvious change does not, by itself, explain why progress is slow. It is a reason to ask your therapist to review the plan and explain the evidence and clinical reasoning behind it.

Key Takeaways

  1. Progress needs a purpose. Your treatment goals should connect to mobility or daily independence.

  2. Progress needs to be assessed. Ask how your therapist knows whether the plan is helping.

  3. Exercises need an explanation. You deserve to understand the neurological rationale behind what you have been asked to do.

  4. Questions are part of good care. Asking for clarity is a practical way to take part in decisions about your rehabilitation.

Implementation Checklist

  • Write down the changes you hope to see in mobility or daily independence.

  • Take the five questions to your next appointment.

  • Ask your therapist to explain any terms you do not understand.

  • Note how progress will be assessed and when the plan will be reviewed.

  • If the answers remain unclear, ask for a further discussion about your treatment plan.

Common Mistakes to Avoid

  • Assuming that attending sessions means you are making progress.

  • Continuing exercises without understanding why they were chosen.

  • Waiting until frustration builds before discussing a lack of tangible change.

  • Treating questions about your care as being difficult.
